How to use the forms:
Quarterly, give each staff member a self-evaluation form and ask them to self-reflect on their performance. Have them self-evaluate what areas they think they can improve in and what areas they think they excel in.
The director or administrator will also do a short observation and evaluate the staff member's performance (both forms ask the same questions).
Pick a day to schedule a one-on-one meeting to discuss both findings and make a plan of action for goals or any improvements needed and celebrate areas of excellence and growth.
